Study Design and Population
The TAIPAN longitudinal cohort study harnessed routine health records from 69 HIV services in Australia's most populous states, New South Wales and Victoria. Covering the period from January 1, 2010, to December 31, 2019, the study enrolled 101,772 gay and bisexual men (GBM), including both HIV-positive and HIV-negative individuals. This large dataset allowed researchers to directly measure HIV incidence and track changes over a decade.
Impressive Gains in Viral Suppression
Over the study period, viral suppression among HIV-positive GBM—defined as a plasma viral load below 200 RNA copies/µL—increased from 69.27% to 88.31%. This steady improvement reflects expanded access to antiretroviral therapy (ART) and better adherence support.
Steep Decline in HIV Infections
HIV incidence among GBM fell dramatically from 0.64 per 100 person-years in 2010 to 0.22 per 100 person-years in 2019—a 66.37% reduction. The analysis revealed that each 1% increase in population-level viral suppression corresponded with a 6% decrease in HIV incidence (IRR 0.94, 95% CI 0.93–0.96; p<0.0001).
The Synergistic Role of PrEP
Pre-Exposure Prophylaxis (PrEP) was introduced in 2016. By 2019, PrEP uptake reached 36.38% among participants. After PrEP became available, the link between viral suppression and HIV incidence strengthened (IRR 0.80, 95% CI 0.70–0.93; p=0.0030), indicating a complementary effect.

Success stemmed from coordinated efforts by government, clinics, and community organizations to eliminate barriers to ART—such as prescribing restrictions and costs—and to expand HIV testing options. The observational design precludes direct causality, but behavioral surveys indicating declining condom use suggest that changes in sexual practices alone cannot explain the incidence drop.
